
Worcester District Collaborative is a group of people and organisations working together at a local level to meet the needs of the communities they serve.
The Collaborative wants to help people to remain healthy and well for longer. It aims to do this by making sure all communities have access to support and services which help them to be well and prevent an increase in the severity of their needs.

Priorities
Worcester District Collaborative has three priority areas. These are based on data about the local health and wellbeing needs of people living in the district.
Tackling loneliness and social isolation
There will be a focus on supporting individuals by raising awareness of available facilities and providing signposting to services.
Providing the best start in life during maternity and children's health
There will be an emphasis on mental health and improving health outcomes which tackle childhood obesity.
Health and Inequalities
Key benefits will include improved access to health checks and screening for prevention and early diagnosis of illness.
